All dogs registered in our program, must be brought into Petsmart at least twice each month. We
showcase our dogs there every Sunday between noon and 4 p.m.
Dogs not brought into Petsmart on a regular basis will be considered withdrawn from our program.
You may reactivate the registration by contacting us.
To help us offset the cost of vet care, transportation, etc. we request a donation as follows to enroll
your dog in our program. All donations are tax deductible and a tax receipt will be issued to the
address on the first page of this form, unless otherwise advised.

We appreciate that giving up a pet for adoption can be a very difficult and emotional
experience. Our volunteers take every effort to ensure that a safe and suitable home is found.
The following information will provide you with knowledge of our adoption procedure, what services TAGS will
provide, and in turn what TAGS will require from you should you decide to enroll your pet in our program.

Ensuring all medical work is completed prior to adoption, example spay/neuter etc.

Your Responsibility to our Program

Complete the Owner information profile, providing facts about your pet to assist the
adoption process.

To provide a tax deductible donation, to help offset any incurred expense in the adoption process.

To bring your pet to Petsmart on Sundays between the hours of 12-4 p.m to increase awareness and for
potential adopters to meet. Should you not be able to bring the dog in, you will call to inform volunteers
or to arrange for pick up.

To return phone calls or e-mails from volunteers promptly and to notify the program of any relevant
changes to the adoption status of your pet. For example, decided to keep your dog.

Provide a complete medical history. Rabies vaccinations are normally only required once within a 3 year
period (Imrab 3). If unsure have our volunteers or your vet confirm.

Ensure that all information provided on the info sheet is not false or misleading, which could result in
harm to a potential adopter and result in the dog being returned to you the owner. Should this occur, you
as the owner, will be responsible for any medical costs incurred by The Animal Guardian Society.
